What is an SNP?

A Medicare Special Needs Plan (SNP) is a type of Medicare Advantage Plan which offers specialized benefits, provider choices, and drug plans to meet best the needs of the groups of people they serve. Enrolling in an SNP is limited to individuals who are enrolled in both Parts A & B of Medicare, live in the plan’s service area, and have specific health conditions or financial limitations. All SNP’s are also required to provide Part D (prescription) coverage, and some even offer benefits such as vision and hearing care.

The two SNP’s discussed here are Chronic Condition (or C-SNP) and Dual Eligible (or D-SNP). Other forms of premium assistance may be available to you if you are eligible for an SNP.

What is Chronic Condition (or C-SNP)?

Chronic Condition Special Needs (C-SNP) Plans are a type of Medicare Advantage (MA) plan designed to meet the unique needs of people with one or more severe or disabling chronic conditions. Some examples of these conditions include:

  • Diabetes mellitus
  • Lung disorders
  • Heart conditions
  • Stroke / Bypass
  • High blood pressure
  • Chronic obstructive pulmonary disease (COPD)
  • Cardiovascular disorders
  • End-stage renal disease (ESRD)

What is Dual Eligible (or D-SNP)?

A Dual Eligible Special Needs (D-SNP) Plans are a special kind of Medicare Advantage coordinated-care plan. It is an all-in-one plan that combines your Medicare Part A and Part B benefits, your Medicare Part D prescription drug coverage, your Medicaid benefits, and additional health benefits such as vision, dental, or fitness.

If your monthly income and resources are below a certain limit, you may also qualify for assistance with your monthly payments of Medicare premiums through something called a Medicare Savings Program. If you qualify, this might also pay your Part A and Part B deductibles, coinsurance, and copayments.  

Seniors and younger adults with disabilities who may NOT qualify for full Medicaid but may still be able to enroll in the Medicare Savings Programs.

When Can I Enroll & What is the Cost?

If you qualify for a Special Needs Plan (SNP), you can enroll for the plan at any time. You can also switch plans whenever you want under these circumstances throughout the year as well.

Your exact costs can vary depending on the plan you choose. Generally, you will continue paying your Medicare Part B premium, though some SNP will pay part, or all, of this premium.

Like other Medicare Advantage Plans, there may be additional monthly premiums for prescription drug benefits or more inclusive coverage. Other forms of premium assistance may be available to you if you are eligible for an SNP.

Also, if you are enrolled in both Medicare and Medicaid, you are considered Dual Eligible and most of the costs of joining a Medicare SNP will be covered at NO additional cost to you
